NDIS Support Coordination | Sydney, Central Coast

Living Strength Occupational Therapy is a registered NDIS provider. We are registered for Assist-Life Stages, Transition and Support [0106] and Support Coordination [0132] in NSW.

Our support coordination services take the stress out of understanding and implementing your NDIS plan. We specialise in interpreting your NDIS plan and end-of-year NDIS plan report. Our support coordination team is led by Barbara Luka, who has extensive experience in this area.

We provide services in the Sydney metropolitan area and on the NSW Central Coast. However, if you’re outside these areas or we can’t meet in person, we will video conference with you as needed.

Role of an NDIS support coordinator

A  support coordinator connects participants who have an NDIS plan to supports and services in their community. We aim to complement your life and help put your NDIS plan into action.

A support coordinator can help you:

  • understand how to use your NDIS plan to meet your goals
  • manage resources effectively to get the best outcome from your plan
  • work with other people in your support circle, such as family, friends, service providers, therapists, and community and mainstream supports like doctors
  • access the NDIS portal so you can monitor how your funds are being spent
  • monitor your progress and outcomes
  • prepare for your next NDIS review meeting

Stages of support coordination

 

Stage 1: Engagement

After we receive the referral request, our admin team will meet with you to discuss your NDIS plan and assess how we can best help you achieve your NDIS goals.

We may ask you to share information about yourself (such as name, NDIS number, disability and people most important in your life) so we can understand what goals you would like to achieve. We will then develop a service agreement that outlines how you and our Living Strength OT team will work together. If you choose us as your provider for support coordination, you will need to sign the service agreement and then our admin team will match you with the best support coordinator for your needs, goals and aspirations.

Stage 2: Planning

Your matched support coordinator will contact you to set up a planning meeting. We encourage your circle of supports to attend this meeting so we can fully understand your life.

During the meeting, we will talk about:

  • your NDIS goals
  • your funded support categories
  • how support coordination works

Together, we will develop a whole-of-life program that is tailored to you and reflects your needs, desires and aspirations. This will help you achieve the goals in your NDIS plan.

After the meeting, we will match you with the support staff of your choosing and services for you.

Stage 3: Implementation

Your support coordinator will meet with you to discuss how your goals are being achieved. They will also help you navigate the NDIS by connecting and coordinating informal, mainstream and funded supports in a service delivery environment. We promise to walk alongside you in your NDIS journey and help make it happen!

Stage 4: Monitoring

Once your supports and services are in place, we will work with you to monitor your goals, needs and aspirations. We will keep records about how your goals are being achieved, which will give you useful information when you review your plan with the NDIA. This usually happens once a year.

Managing your own plan

As we work with you, we will also develop and strengthen your skills and confidence in taking control of your NDIS plan. Once you’re comfortable coordinating your NDIS plan, you may choose to manage your own plan independently.

We can help you to:

  • take over some of your case coordinator functions over time and request the things you need
  • manage your own plan independently

This is an individual process and everyone learns these skills in their own time. We won’t rush you in this process or force you to take on tasks you’re not comfortable with.
